The hospital discharge process

The hospital discharge process is a critical juncture in a patient's healthcare journey. It marks the transition from hospital-based care to continued recovery and support, often at home. At Nurseplus Care at home, we understand the importance of this transition and are here to guide you through the hospital discharge process with our experienced team members.

Each hospital will have it's own discharge policy. You can ask for a copy from the Ward Manager or Patient Advice and Liaison Service. Nevertheless, planning around a person’s hospital discharge should take place as soon as they are admitted to the hospital.

If you are the primary carer for a person in the hospital, then it is important to notify the hospital as early as possible that you plan to care for them after their discharge. This helps ensure that the healthcare team will involve you in the process of planning hospital discharge care.

If it is decided that care is required after your hospital discharge, then a hospital discharge assessment will be carried out by a team of health and social care professionals to identify any specific needs you may have.

The assessment carried out will identify your care requirements along with eligibility for NHS funding.

What types of care are available at hospital discharge?

• Reablement care: also known as rehabilitation care or reablement services, is a short-term and intensive approach to homecare designed to help individuals regain their independence and daily living skills following an illness, injury, or hospitalisation. The primary goal of reablement care is to support individuals in relearning or improving the skills necessary to perform essential daily activities and routines on their own. This form of care aims to reduce an individual's reliance on ongoing care services and promote their ability to live independently again.

• NHS Continuing Healthcare (CHC): is a type of care for people with long-term complex health needs who will qualify for free social care arranged and funded solely by the NHS. More information on NHS continuing healthcare, including eligibility, can be found here.

• Private or self-funded care: self-funding, also known as private care, is a popular option for individuals who have the financial means to pay for their own homecare services. With self-funded care, you have the flexibility to tailor your care plan according to your specific needs and preferences. This option allows you to choose from a wide range of homecare providers, such as Nurseplus Care at home, and customise the level of assistance required.
